Lumbar Transforaminal Epidural Steroid Injection
A Lumbar Transforaminal Epidural Steroid Injection (TFESI) is a targeted, minimally invasive procedure that delivers anti-inflammatory medication directly to a specific spinal nerve root in the lower back. This focused approach helps relieve pain caused by nerve compression or inflammation from conditions such as herniated discs, spinal stenosis, or sciatica. By reducing swelling around the affected nerve, TFESI can decrease pain that radiates into the buttocks, hips, or legs and improve mobility and quality of life.
Quick facts
- Setting: Outpatient, image-guided
- Time: ~20–30 minutes
- Anesthesia: Local with optional light sedation
- Recovery: Same-day discharge; mild soreness possible for 1–2 days
- Duration of relief: Weeks to months; repeat injections may be recommended
Who it helps
- Lumbar disc herniation or bulging disc
- Spinal stenosis (narrowing of the spinal canal)
- Foraminal narrowing or nerve root compression
- Sciatica or radiculopathy (leg pain, tingling, or weakness)
- Chronic lower back and leg pain unrelieved by conservative treatments
How it works
In the lumbar spine, nerve roots exit through small openings called foramina. When a disc bulges or degenerates, these nerves can become inflamed or compressed, causing pain. During a transforaminal epidural steroid injection, your provider uses fluoroscopic (X-ray) guidance to precisely insert a thin needle into the targeted foramen next to the affected nerve root. A combination of corticosteroid and anesthetic medication is then injected to reduce inflammation, relieve pressure, and calm nerve pain.
What to expect
- Before: Your provider will review your medical history and imaging. You may be advised to hold certain medications (such as blood thinners) before the procedure.
- During: You’ll lie comfortably on your stomach. After numbing the skin, a small needle is guided under X-ray into the foramen near the irritated nerve. Once placement is confirmed, the medication mixture is slowly injected.
- After: You’ll rest briefly in recovery and can usually go home the same day. Some temporary numbness, tingling, or soreness is normal. Pain relief typically begins within several days.
Benefits & risks

FAQs
- When will I feel relief? Most patients notice improvement within 3–5 days as inflammation decreases.
- How long does relief last? Relief may last several weeks to months; repeat injections may help extend results.
- Can it prevent surgery? For many patients, TFESI helps manage pain effectively and may delay or eliminate the need for surgery.
- Is it painful? The injection is well tolerated; local anesthesia minimizes discomfort.
- Is it covered by insurance? Most major insurance plans, including Medicare, cover this procedure when medically necessary.
